Our story

Emergency-trained care, neighborhood scale

Our clinics grew out of decades spent in busy emergency departments, where a large share of patients did not need an emergency room — they needed to be seen the same day by someone who could order an x-ray, run a lab, and make a decision.

That is the model we run in Bonsack, Cave Spring, and Westlake. Walk in, be examined by a provider, get testing on site, and leave with treatment underway. No appointment, no referral, no maze.

We are locally operated and we live here. The families we treat are neighbors, coworkers, and the people behind us in line at the grocery store — a good reason to keep the standard high.

Founder & Owner

David B. Alligood, MD

Dr. David B. Alligood, founder of Express Family Care, is a graduate of Wake Forest University School of Medicine who completed his internship and residency in Roanoke, where his fellow residents and faculty elected him Chief Resident. A board-certified family physician, he has practiced medicine continuously since 1993 — first in hospital emergency departments across the region, and since 2013 as founder and Medical Director of Express Family Care, which grew from the original Bonsack clinic into three locations serving the Roanoke Valley, Bedford County, and Smith Mountain Lake.

He has been voted a “Top Urgent Care Doctor” by Roanoker Magazine in 2023, 2025, and 2026, and named Best Bedside Manner by Our Health Magazine five times. That emergency background shapes how our clinics operate: rapid triage, on-site testing, and a low threshold for imaging when something does not add up.

Beyond the clinic, Dr. Alligood has volunteered on medical mission trips to Honduras, Ukraine, Haiti, Israel, and El Salvador, and has taught Advanced Trauma Life Support since 2004.

Dr. Alligood provides medical direction across the Bonsack, Cave Spring, and Westlake clinics.

Dr. Alligood and his wife make their home on Smith Mountain Lake. They enjoy traveling and spending time with their five adult children.
